A man needed stitches to his face after being attacked with a roof tile.
Police say the 35-year-old victim was talking along Central Avenue towards Prestatyn town centre when he was approached from behind by another man.
The incident happened between 8.20pm and 8.30pm on Saturday, April 9.
An appeal for witnesses has now been launched.
The offender is described as being white, in his early 20s and had very short dark hair.
He was wearing a grey or white coloured hoody and tracksuit bottoms. He left the area in a light blue coloured Toyota Yaris which travelled along Central Avenue and then turned right up Ffordd Penwylfa.
PC James Lloyd said: “This appears to have been a pre-planned attack which has left a man with serious facial injuries. I am appealing to anybody who may have information, or to anybody who may have witnessed the incident to contact us immediately.”
Police enquiries are ongoing and anybody with information is asked to contact PC 3155 James Lloyd at Rhyl Police Station on 101, quoting reference number RC16051042.
