Prosecutors previously advised not charging Kris Wade following allegations he had abused vulnerable patients - three years later he murdered grandmother Christine James

A healthcare worker was free to murder a grandmother after prosecutors advised not charging him following allegations he had abused vulnerable patients.
Kris Wade, 37, allegedly attacked three women with learning disabilities on a unit where he worked.
South Wales Police say they questioned Wade voluntarily about the claims but dropped the case on the advice of the Crown Prosecution Service, the Mirror has reported .
Three years later Wade murdered Christine James, 65, a businesswoman who lived below him in Cardiff .
He was jailed for life last week with a minimum term of 21 years after pleading guilty to the March attack.
A review has now been launched by the health board that employed Wade in Cardiff.

Police 'investigated three separate allegations'

Wade was never arrested despite the separate victims showing a ‘similar pattern’ of offending.
Instead he was questioned under caution by South Wales police in 2013.
Wade was allowed to return working with vulnerable patients after the first victim came forward in 2012.
He was suspended again later that year when the second victim came forward.
He was then sacked earlier this year after being found guilty of misconduct.
The CPS said it could not confirm a file had been received because documents are destroyed if cases are not pursued.
A police spokesman said: “Between 2012 and 2013, South Wales Police investigated three separate allegations that patients in Cardiff had been sexually assaulted by a member of staff.
“As part of those investigations, a man who was in his 30s at the time, was voluntarily questioned by officers. In each case no further action could be taken following advice from the Crown Prosecution Service.”

A spokeswoman for Abertawe Bro Morgannwg University Health Board said: “We can confirm that Mr Wade was dismissed from his job as a healthcare support worker following allegations from three patients in Cardiff that he had sexually assaulted them.
“We have offered our profound apologies to the patients involved, and the distress his actions undoubtedly caused.”
She added: “We will now be reviewing our processes to see if there are any lessons to be learned.”
